Hypertension and Pheochromocytoma – An Overview
Although it accounts for less than 1% of cases, pheochromocytoma is important to distinguish from other causes of hypertension. Unrecognised, the disease can be fatal. Pheochromocytoma is a catecholamine-secreting tumour that arises from adrenal medullary or sympathetic ganglion tissue.
Pheochromocytoma produces an excess of adrenalin, noradrenalin, dopamine and their methoxy derivatives metanephrine, and normetanephrine. Diagnosis is usually made by the detection of abnormally high levels of one or more of these substances in urine or plasma. Despite a variety of disadvantages, for the last twenty years measurement has been mainly by High-Performance Liquid Chromatography (HPLC). This technology is usually restricted to use in large laboratories due to the prohibitive cost of installation, maintenance and training—not to mention the ongoing technical challenges.
